TS 3.3.5 ESFAS Instrumentation Flashcards
LCO 3.3.5
___ ESFAS trip and bypass removal channels for each Function in Table 3.3.5-1 shall be OPERABLE
APPLICABILITY: ___
Four
APPLICABILITY: According to Table 3.3.5-1
LCO 3.3.5
ACTIONS
NOTE: ___
NOTE: Separate Condition entry is allowed for each ESFAS function
Table 3.3.5-1 ESFAS Instrumentation
What are the 7 FUNCTIONS?
And WHEN are they Applicable?
SIAS CSAS CIAS MSIS RAS AFAS-1 & AFAS-2
All are Applicable Modes 1, 2 and 3
LCO 3.3.5 CONDITION
A. 1 or more Functions with 1 Auto ESFAS trip channel inoperable
A.1 Place channel in bypass of trip (1 hour)
AND
A.2 Restore channel to OPERABLE status (Prior to M2 entry following next M5 entry)
LCO 3.3.5 CONDITION
B. 1 or more Functions with 2 Auto ESFAS trip channels inoperable.
B.1 Place 1 channel in bypass and the other in trip (1 hour)
LCO 3.3.5 requires ___ channel components necessary to provide an ESFAS actuation to be OPERABLE.
ALL
Example: CTMT Pressure - High (has 4 channels A-D)
For PZR Pressure-Low:
When the trip set point has been lowered below the bypass permissive set point of ___, the PZR Pressure - Low reactor trip, CIAS, and SIAS actuation may be manually bypassed in preps for SDC.
400 PSIA
- This is an example of channels of Bypass Removal that are ALSO required per this TS (LCO applies to bypass removal feature ONLY)
The most common causes of channel inoperability are ___
Outright Failure or Drift of the bistable or process module sufficient to exceed the tolerance allowed by the plant specific set point analysis.
LCO 3.3.5 CONDITION
C. One or more Functions with one automatic bypass removal channel Inoperable.
C.1 Disable bypass channel (1 hour) OR
C.2.1 Place affected auto trip channel in bypass or trip (1 hour) AND
C.2.2 Restore bypass removal channel and associated auto trip channel to Operable status (Prior to M2 following next M5 entry)
LCO 3.3.5 CONDITION
D. One or more Functions with two automatic bypass removal channels inoperable
D.1 Disable bypass channels (1 hour) OR
D.2 Place one affected auto trip channel in bypass and place other in trip (1 hour)
If the bypass removal channel for any operating bypass cannot be restored to OPERABLE status, the associated ESFAS channel may be considered OPERABLE only if ___
The bypass is NOT in effect
If one ESFAS channel is in TRIP and a second channel is in BYPASS, a third inoperable channel would place the unit in ___
3.0.3
LCO 3.3.5 CONDITION
E. Required Action and associated Completion Time not met.
E.1 Be in M3 (6 hours) AND
E.2 Be in M4 (12 hours)
LCO 3.3.5 - SR 3.3.5.5 - Perform a CHANNEL FUNCTIONAL TEST on each auto bypass removal channel.
Once within 92 days prior to each RX startup
Per LCO 3.5.3, Condition B, One of the two inoperable channels will need to be restored to OPERABLE status prior to the next required CHANNEL FUNCTIONAL TEST because ___
…channel ST’s on an OPERABLE channel requires that the OPERABLE channel by placed in bypass. However, it’s not possible to bypass more than one ESFAS channel, and placing a second channel in trip will result in an actual ESFAS actuation.
