Same as a secondary survey

Trachoestomy - through the cricothyroid membrane
How do you assess diability?

Pupils (PEARL - pupils equal and reactive to light)
How do you assess circulation?
Pulse
BP
Temp/cap refill of peripheries
Bleeding
When do you do the tertiary survey? Why?
24 hours after
To pick up things possibly missed, hopefully while the patient is conscious
What is the alternative to IV assess?
Interosseous
What do you do in the secondary survey?
Head
C-spine
Chest
Abdo/Pelvis
Upper limb
Lower limb
Groin
What are some other adjuncts to the primary survey?
ABG - Hb
Log roll + anal tone
Catheterisation and urinalysis
What is the acromin used for history taking in trauma?
A - Allergies
M - Medx
P - PHx
L - Last meal
E - EtOH/Drugs
Where do you put a ICC? What size tube do you use?
5th intercostal space in the mid axillary/ant axillary line
(or at the nipple line males and where the nipple line should in a female)
32 French (or 28 French in a small female)
What is a pan scan?
CTs
Non-contrast head and neck
Contrast chest angiogram
Venous phase abdo/pelvis
What are reasonable mechanisms for transection of aorta?
MVA >80km/h
Motorbike accident >60km/h
Hit by car at >40km
>3m fall
How do you assess breathing?
SaO2
RR
Auscultate - mainly for the present of air entry
Symmetry of chest rise and fall
What are the adjunct imagining to the primary screen?
CXR
Pelvic Xray
