CAGs and Stents Flashcards
Which internal mammary artery do you use? Why?
L because the R has to transverse the heart
What are common graft sites? Which arteries due their repair?
LIMA - left internal mammary artery (one of the first branches of the aorta) for LAD
RA - Radial artery for RCA or LCircumflex
SVG - Saphenous venous grafts for PDA
What are the types of stents?
Bare metal stents
Drug eluting stents
What are the branches of circumflex?
L marginals
- OM1, OM2
Terminates at the crux
Where does the RCA run?
Arterial/ventricular groove
What is the name of test for ulnar artery patency?
Alan’s test
What tissue layers are cut through in a stenotomy?
Skin
Sternum
Thymic fat
Pericardium
What are the branches of the LAD?
Diagonals - D1, D2, D3
Septal
What are the branches of the RCA? And their supply and course?
SA nodal artery
Right atrial aa.
R anterior ventricular (RV and RA)
R Marginal (RV, runs on the interferior border of the heart towards the apex)
Posterior descending artery
AV nodal branch
Describe the course of the LCA?
Origins at the left leftlet
Up and behind the left coronary apendage
and bificates
Where does the R coronary artery arrise from?
Adjacent to the R leftlet of the aortic valve
Which drugs are used for post op arrhythmias?
Metoprolol > digoxin > amiodarone
What are the common post operation risks for CABGs?
Bleeding, infection
Arrhythmia - AF and atrial flutter
Amnesia
Pain
Dyspnoea, atelectasis, pneumonia
Numbness
MI, stroke, death
When do you use PCI vs CAGs?
Single vessel disease - PCI
RCA and circumflex - PCI
LAD and either RCA or circumflex - CABG
Three vessels - CABG
