Mr B 72y small CVA. Underwent CABG/Carotid stenosis.
Suffered periop hemispheric stoke. Difficult to wean from MV & has poor gag reflex.
Neurologist opinion is need high level nursing home care should he survive hospitalization.
Surgeon wants to do tracheotomy.
Wife is unhappy with idea of tracheotomy.
Negotiation with surgeon before speaking to wife.
Establish if wife is operating to defend her hsband best interest.
Wife - may fell trachesotomy is an extreme measure to preserve a life which her husband would not want OR
hoping he will get better and not want to undertake tracheostomy at this early stage.
Surgeon - strong feeling preserve his patient life & the iatrogenic nature of the event OR he just feel tracheostomy is next medical step.
Possible emotions
Wife angry with complication of sx.
Extreme measure to preserve life OR hsbnd not that sick & will soon be fine.
