Gently and sympathetically break the bad news
Clearly explain what has happened
Express regret that the error has occurred & death has resulted
APOLOGIES/ APOLOGIES>APOLOGIES
Explain that an investigation will occur
Give reassurance that the family will be informed of the outcome and supported
Show empathy
Accept the anger is natural and let this occur
I AM AFRAID THAT A DREADFUL THING HAS HAPPENED
UNFORTUNATELY THIS WAS VERY, VERY SERIOUS AND I AM TERRIBLY SORRY TO TELL THAT UR MOTHER HAS DIED
I CAN UNDERSTAND Y U R ANGRY ABOUT THIS
THE BREATHING TUBE THAT SHE DEPENDED ON CAME OUT. AT THIS STAGE I DON’T KNOW WHY THIS HAPPENED. IT SHOULD NOT HAVE HAPPENED.
I AM REALLY SORRY THAT IT HAS OCCUERED - AND OBVIOUSLY THE EFFECT IS ABSOLUTELY AWFUL
WE WILL FIND OUT EXACTLY WHAT HAS HAPPENED & U WILL BE KEPT INFORMED
WE NEVER EVER WANT THIS TO HAPPEN AGAIN.
