Indications
A
- Acute ischaemic stroke
- In acute ST elevation myocardial infarction
- For massive pulmonary embolism with haemodynamic instability

Mechanism of action
A
Catalyse the conversion of plasminogen to plasmin, which acts to dissolve fibrinous clots and re-canalise occluded vessels.

Adverse effects
A
Nausea
Vomiting
Bruising

Warnings
A
Contraindicated in recent haemmorhage, recent trauma or surgery, bleeding disorders, severe hypertension.
Intracranial haemorrhage must be ruled out via CT before treatment.

Interactions
A
Increased risk of bleeding when used with other anti-coagulants.

Prescribing
A
Should only be used by those experienced in their use.

Monitoring
A
Pts should be monitored in an HDU/ICU setting.
