Deaths must be reported to the coroner if they are:

A
S	= Suicide
C	= Cause unknown or no Doctor’s Certificate issued
U	= Unnatural or violent death
M	= Medical or surgical or dental operation or procedure
B	= Birth deaths, or deaths that appear to be the result of pregnancy or giving birth.
I	= Institutional deaths; deaths in a psychiatric hospital,or prison, or while in police custody.
E	= Enactment requires the holding of an inquest (CYPFA, Corrections, Armed forces etc.)

E = Reduce / ELIMINATION the chances of the occurrence of deaths in similar circumstances.

What is the function of the coroner?

A

From the inquest to:

Investigate into the facts, causes,and circumstances of the death.

Make recommendations or comments that reduce the chances of other deaths occurring in similar circumstances to those in which the death occurred.

Determine whether the public interest would be served by the death being investigated by other investigating authorities, and to refer the death to them if satisfied that the public interest would be served.

It is NOT to determine civil, criminal or disciplinary liability.
