What is COBRA?
A continuation of Plan coverage, when it would normally end due to life event (death).
How can eligible spouse/dependent become qualified for COBRA?
- death of spouse/parent
- spouse work hours drop
- spouse employment ends for reason other than gross misconduct
- spouse gets Medicare
- divorce/separated
Duration of COBRA coverage?
- up to 18 months due to end of employment/less hours
- up to 36 months due to death/divorce
- for 36 months after date of Medicare
When is payment for COBRA coverage due?
NLT 45 days after date of election.
Who is enrolled in the AZ State Ret System or the PSPRS?
Any employee/temp working at least 20 hrs weekly for 20 weeks or more in a fiscal year.
What is Deferred Comp Plan?
Governmental 457 plan, is a retirement savings plan that allows employees to supplement existing retirement/pension benefits. (tax deferred until withdrawn).
What is Bereavement Leave?
Immediate family member death.
-40 hrs for reg status employee
-52 hrs for 52 hour per week employee
(any extra hours charged against sick/vacation leave)
Civic Duty Leave?
-a 2000 hour pool annually for all employees to draw from, to perform civic/community duties. (paid time off).
Who is eligible for Education Leave?
Regular status employees who work 40 hrs plus per week and completed 1 full year service, are allowed 20 hours per year (must be job related).
What is Election Day Leave?
3 consecutive hours immediately after polls opening, also prior to polls closing.
What is Exceptional Disability Leave?
Regular status employee who works 40+ hrs weekly, is unable to work due to injury/illness, and needs very long period of recovery, can apply (must be out of sick/vacation/comp time).
How many hours can an employee receive for Exceptional Disability Leave?
- 5 - 10 yrs of service = 240 hours
- over 10 yrs service = 400 hrs
What is FMLA Leave?
Gives employees up to 12 weeks of job-protected leave per year.
How is an employee eligible for FMLA?
Must have worked 12 months, and worked minimum 1250 hrs over past 12 months.
How are 2 married members employed by city, entitled to FMLA?
They are Jointly entitled to 12 weeks.
When are employees expected to schedule FMLA Leave?
At a time when it least disrupts City Operations.
What is employee responsibility with FMLA?
- give all documentation the City needs
- give as much notice as possible to Supervisor
- 15 days to get medical/supporting doc to HR/RM
- complete timesheet
- report status to Supervisor and HR/RM
What is Supervisor responsibility with FMLA?
- comply with HIPAA for employee privacy
- notify HR/RM when employee in hospital for 1+ days or unable to work for 3 consecutive days
- submit Personnel Action Form to HR/RM
- may update employee’s timesheet
How is Holiday Special Leave given?
For full-time reg status employee:
- 40hr work week gets 12 holiday hours
- 52hr work week gets 15.6 holiday hours
What is Industrial Leave?
Leave required for treatment/recuperation, from injury or illness gotten in course of employment.
When does Industrial Leave coverage begin?
Coverage (worker’s comp) begins on 1st day of employment (no cost to employee).
What are employee reporting responsibility, when injured on the job?
- report injuries to supervisor NLT end of shift/beginning of next shift
- immediately call 24/7 Nurse Hotline to report injury
- go to City Med Center or give supervisor and RM the name, number of personal treating physician
- schedule appts when it least disrupts City operations
- get Work Status Report at every doctor’s visit
- accept modified duty as offered by city (based on City need)
Leave Without Pay?
- requests go to department director
- requests must be made for 1 or more days (normally won’t exceed 1 full work week in a 2 week period)
When can Medical Leave Of Absence be granted?
-when employee doesn’t qualify for FMLA
-when FMLA is exhausted
(leave is unpaid, can be used once sick/vacation is exhausted)
What is the max number of hours an employee can use for Military Leave?
- 320 hrs per fiscal yr (40 hr wk employee)
- 416 hrs per fiscal yr for fire (52 hr wk employee)
- *if called to duty, can be reemployed if performed 5 yrs or less cumulative service time
How is Sick Leave accrued?
All regular status employees accrue .055 hrs of Sick Leave benefits per paid hr (not accrued for overtime).
When does eligibility of Sick Leave benefits commence?
After completion of 30 days regular status employment.
Can the city verify Sick Leave?
- the City may require doctor’s verification for sick leave in excess of 3 days
- City can refer employee to any city-designated doctor, to determine whether Sick Leave will be paid
When can employees exercise the “Sick Leave Cash-In Option”?
During April and October of each year (cash-in month)
How will Sick Leave Cash-In be paid?
1/3 of average hourly wage, for last 36 months.
How is vacation accrued?
On a per hour basis.
- less than 5 yrs = .066625
- 5 to 10 yrs = .0765
- 10 plus yrs = .095
What are stipulations for donating leave?
- from regular status employ to regular status employee
- employee donating must have 40 hrs remaining afterward
- fill out a Vacation Leave Donation form
- once approved, PA Preparer will send to HR/RM for final approval
- HR/RM sends to Payroll for processing
What employees are covered by the City’s EAP program?
Reg status employees working 20hrs or more weekly.
covers counseling, financial/legal services
How long can retirees opt to continue their Basic Life Insurance policy?
Up to age 70 (under group rate).
How is request for Civic Duty Leave submitted?
Submitted to supervisor NLT 5 days in advance.
Is FMLA Leave paid?
FMLA Leave is unpaid leave, but if you have sick/vacation time, you may use ALONG WITH FMLA so you continue to get paid.

- Investigate
- complete Incident Notification Report within 24 hrs
- ensure employee reports injury to 24/7 Nurse Hotline
- ensure employee gets work status report
- keep informed of employee work status/restrictions
- complete Personal Action Form (PA)
- enter/approve all Time and Labor time codes
What is difference between Sick Leave Payout at Resignation, and Sick Leave Payout at Retirement?
Resignation: with 5 or more yrs, paid 1/3 of hourly wage (average over last 36 months)
Retirement: with 10 or more years, paid into Retiree Health Savings account.
What is Education Assistance?
Reg full-time employee receives education assistance in form of one-time, lump sum to offset costs.
