Symptoms of TB (active)
Sob
Blood in sputum
Fever over 2 weeks
Chest pain
Cough
Weight loss
Children problems in growing
Night sweats
Loss of appetite
What are the signs in latent TB
Not shown as early so body fights it off. If body can’t fight it off then becomes active TB
Phase 2 TB treatment
RI for 4 months
Initial phase treatment TB
2 months long
Rifampacin
Isonazid (inhibitor so give b6 pyridoxine with)
Pyrazinamide
Ethambutol
Phase 2 TB treatment
RI for 4 months
What is rifampacin
Inducer more potent so wins
What is isonazid
Inhibitor so give pyridoxine (b6 with it) to prevent neuropathy
Side effects of rifamapcin
Red or orange urine or contacts or any liquids
Take on empty stomach
Hepatotoxic
Thrombocytopenia
Psychosis
Side effects of isonazid
Inhibitor so causes vitamin B6 deficiency
Neuropathy -Give pyridoxine with it
Liver toxicity
Ethamvutol side effects
Vision disorders
Pyrazinamide side effects
Liver toxicity
What side effect do all ripe drugs have
HEPATOTOXICITY = monitor liver before treatment
How long is second continuation phase of TB
4 months treatment in active TB without neurological cns involvement
10 months if active tb with cns involvement and offer dexamethasone or prednisone and withdraw slowly over 4-8 weeks
What is classed as treatment interruption in Tb initial phase
More than 2 weeks gap or missing more than 20% of doses
If due to hepatotoxic then resume once ALT back to normal and cont treatment max 10 days
Caution of isonazid
Diabetes
B6 deficiency
Alcohol dependence
Acute porphyria
Epilepsy
HIV
Malnutrition
