Multi-system autoimmune disease
What is the pathophysiology behind SLE?
Inadequate T cell suppressor activity with increased B cell activity
B cells secrete auto-antibodies
What gene is involved in SLE?

What environmental factors can bring on SLE?
UV light
Drugs
Infection
What is the pneumonic for symptoms?

What are the symptoms?
Serositis
Oral ulcers
Arthritis
Photosensitvity
Blood disorders - low WCC, lymphopenia, haemolytic anaemia Renal involvement - glomerulonephritis Auto-antibodies - ANA Immunology - low c3, c4 Neurologic disorder
Malar rash
Discoid rash
What is a malar rash?
Red rash across cheeks
- tends to spare nasolabial folds
What investigations do you do for suspected SLE?
Bloods
- FBC
- CRP
- U+Es
- ANA
- anti - dsDNA
- anti phospholipid antibodies
What is ANA?
Anti-nuclear antibody
What are the anti-phospholipid antibodies?
Anti-cardiolipin
Lupus anticoagulant
What is anti-phospholipid syndrome?
Presence of anti-phospholipid antibodies
- either primary
- or secondary to SLE
What is the risk of anti-phospholipid syndrome?
Coagulation deficit
Liverdo reticularis
Obstetric complications
Thrombocytopenia
CLOT
What is liverdo reticularis?
Mottled appearance of thighs
How is anti-phospholipid syndrome managed?
Aspirin
Warfarin
How is SLE managed?
Avoid sun
Screen for HTN, lipids, diabetes
Hydroxychloroquine
Methotrexate
Azathioprine
