Spleen Flashcards
Spleen lies along the axis of
9th to 11th ribs
Why spleen mostly enlarges medially
The spleen mostly enlarges medially since downward displacement is prevented by phrenicocolic ligament.
splenico phrenic ligament and Splenico colic ligament are
Avascular . Can be cut during splenectomy without fear of bleeding
Gastrosplenic ligament.
Short gastric vessels , must be ligated during splenectomy
Lieno renal ligament association
Splenic vessels & tail of pancreas.
Function of spleen
- Immunological response: Production of antibodies.
- Filter
- Pitting (its ability to remove a solid particle from the cytoplasm of a red cell without destroying the cell itself)
- Hematopoiesis
What is Spleniculi
Accessory splenic tissue.
mc site of spleniculi
Hilum of the spleen.
Association of spleniculi
Applied: When splenectomy is being done for hematologic conditions like ITP (idiopathic Thrombocytopenic Purpura), thalassemia or lymphoma, the spleniculi must also be removed as it can lead to recurrence.
Wht is splenosis
Following trauma, splenic particles get deposited in the omentum, bowel and gradually grow into a tumor like mass.
CT Abdomen: multiple nodules all over the abdomen.
Biopsy: Normal splenic tissue.
mc visceral artery aneurysm is
Splenic Artery Aneurysm.
Forms secondary to pancreatitis (MC), trauma or atherosclerosis (elderly patients).
Clinical signs of ruptured splenic artery aneurysm
• Abdominal pain.
• Hemoperitoneum.
• Positive kerr sign
What is Positive kerr sign
Lift left lower limb - Blood will go beneath left dome of diaphragm -> Referred pain to left shoulder tip.
What is splenic infarct
Patient with hypersplenism with enlarged spleen
Mgt of splenic abscess
Unilocular : Pig tail catheter insertion
Multilocular ; splenectomy
