Spinal Conditions Flashcards
why does the thoracic side not see many clinical issues
because it is so rigid and not flexible
in trauma situations what areas of spine must you image?
areas of transition
is the T spine kyphotic or lordotic?
kyphotic
is the L spine kyphotic or lordotic?
lordotic
what does it meant to be kyphotic?
curve out
what does it mean to be lordotic?
curve in
what commonly happens at kyphotic/lordotic transitions?
fractures from high energy trauma
disc degeneration leads to what three things?
loss of height with desicccation and degeneration
what is desiccation of a disc?
loss of water content
are discs vascular or avascular?
avascular
can normal disks herniate?
nah not really
do completely degenerative disc herniate?
no because no nucleus left to herniate
what do you have to have to have a disc herniation?
some degeneration in the annulus
what is HNP
herniated nucleus pulposis
what are symptoms of cauda equina syndrome
acute back pain, sensation changes and urinary retention from large central disk herniation
define cauda equina syndrome
acute neurologic syndrome resulting from acute impingement on nerve roots
what is treatment for caudaequina syndrom?
surgical decompression
name four things that can cause cauda equina syndrome?
disk herniation
tumor
hematoma
abscess
what is sensory change called in cauda equina syndrome?
saddle anesthesia
acute non specific low back pain usually does what over the coming weeks?
resolve…90% by week 12`
what are seven things that make you consider imaging with acute low back pain?
patient older than 50 neuro changes trauma cancer IV use immunosuppressd fever night pain
if patient has history of cancer with new onset of LBP, what do you do?
have to image
does a tumor like the disc spaces in back?
no
does an infection like to get in the disc spaces in the back?
yes
what is the cause of spinal stenosis?
degenerative decrease in the size of the spinal canal
what motion relieves spinal stenosis?
flexion of spinal forward
what is the cause of the pain in spinal stenosis?
chronic encroachment on space available for nerve roots
where do you have pain in spinal stenosis? is it activity related?
lower limb…yes activity related
what pronounces pain with spinal stenosis?
spinal extension
is claudication worse with activity?
yes
what is different between vascular claudication and spinal stenosis when it comes to walking?
for spinal stenosis as long as leaning forward then walking isnt a problem…but with claudication any walking hurts
