How does the addictive process work at the Ventral Tegmental Area (VTA)?
VTA:
• GABA neurons => reward through the tegmental pedunculopontine (TPP) nucleus in mesolimbic system
- Dopamine neurons => aversion signals through nucleus accumbans
- Normally GABA inhibits dopamine release
Addiction:
• When you smoke you undo the inhibition of GABA on dopamine and dopamine gets released
• Overstimulation causes a decrease in dopamine production
• when you stop smoking you produce so little dopamine that you feel cravings and withdrawl
What receptors are associated with reward?
• which are associated with reward and aversive effects?
- Presynaptic alpha7 receptors lead to TPP stimulation (pure reward)
- Post-synaptic alpha4beta2 receptors are associated with BOTH reward and adverse effects

Contraindications:
• with patch watch out for: underlying CV disease (especially if you smoke while on the patch), diabetes, hepatic impairment, peptic ulcers, pheochromocytoma
