Obstructive sleep apnea hypopnea; central sleep apnea; sleep-related hypoventilation.
Obstructive sleep apnea
EVIDENCE ON POLYSOMNOGRAPHY OF AT LEAST 5 OBSTRUCTIVE APNEAS OR HYPOPNEAS PER HOUR OF SLEEP ALONG WITH NOCTURNAL BREATHING SYMPTOMS OR DAYTIME SLEEPINESS; OR ; EVIDENCE BY POLYSOMN. OF 15 OR MORE OBSTRUCTIVE APNEAS OR HYPOPNEAS PER HOUR OF SLEEP REGARDLESS OF ACCOMPANYING SYMPTOMS; specify severity - mild (apnea/hypopnea index <15), moderate (index 15-30), severe (index >30).
Central Sleep apnea
FIVE OR MORE CENTRAL APNEAS PER HOUR OF SLEEP; specify idiopathic, Cheynes-Stokes breathing (crescendo-decrescendo variation in tidal volume), central sleep apnea comorbid with opioid use.
Sleep-related hypoventilation
NO DURATION; polysomnography shows episodes of decreased respiration assoc. with elevated CO2 levels. THING PULMONARY DISEASE, OBESITY; COULD BE CONGENITAL OR IDIOPATHIC.
Parasomnias
nightmare d/o, sleep terrors, sleepwalking d/o, REM sleep behavior d/o, restless leg syndr., substance/medication-induced sleep d/o(??)
Nightmare disorder
NO DURATION; repeated extended extremely dysphoric and well-remembered dreams usually involving threats to survival; USUALLY OCCUR DURING SECOND HALF OF MAJOR SLEEP EPISODE;specifiers including, “during sleep onset, with associated non-sleep disorder, with associated other medical condition, with associated other sleep disorder.” acute/subacute/persistent = <1 month, 1-6 months, >6 months. Specify mild/moderate/severe based on frequency, <1 weekly, one or more episodes per week, episodes nightly.

NO DURATION; OCCURS DURING NREM. NO RECOLLECTION ON PART OF PT; DIFFICULT TO WAKE.
Sleepwalking d/o
NO DURATION; OCCURS DURING NREM. NO RECOLLECTION ON PART OF PT; DIFFICULT TO WAKE.
Rapid Eye movement sleep behavior d/o
NO DURATION; repeated episodes of arousal during sleep assoc. w/ vocalization or complex motor behaviors during REM (at least 90 min after sleep onset); upon awakening, the individual is completely awake.
Restless legs syndrome
DURATION - SYMPTOMS OCCUR AT LEAST 3X PER WEEK FOR AT LEAST 3 MONTHS.
Substance/medication-induced sleep d/o
NO DURATION - name the substance (i.e. caffeine / alcohol / cannabis, opioid, etc.).
