Reposition patient…
at least every 2 hours to minimize the risk of developing pressure ulcers
Shearing force
sliding of skin layers on each other
High risk for skin breakdown from exposure to moisture..
patients who have episodes of incontinence
Stage 1 pressure ulcers
redness that doesn’t return to skin usual color when you apply pressure with a finger
Stage 2 pressure ulcers
a blister with surface skin that peels or cracks open
Stage 3 pressure ulcers
has lost the skin layers and underlying fat and tissue are visible
Stage 4 pressure ulcers
resembles a crater with damage all the way through to muscle and bone
Look for:
dryness, irritated areas, and maceration (softening and breaking down of skin due to moisture)
Supine
lying flat on the back, used for resting and sleeping
Prone
lying flat on the anterior surface of the abdomen, used for taking pressure off the spine and hips
Lateral
lying on one side, used for resting, sleeping, taking pressure off the spine
Fowlers
lying on the back with the head of the bed elevated to various degrees (no more than 30) , used for resting, reading, receiving care, eating, and watching tv
Sims
lying on one side with the knee on that side flexed and in front of the other leg with the knee elevated, relieving pressure on the hips and tailbone having some care procedures
Sitting
upright in a chair with the knees at a 90 degree angle and the feet on the floor or on wheelchair footrests, used for multiple uses
