serious illness Flashcards
A
suction
airway manouvres
airway adjuncts: oropharyngeal or nasopharyngeal airways
enddotracheal intubation
surgical airway
assessment of breathing
effort: resp rate, signs of increased WOB
efficacy: air entry, chest expansion, O2 sats
effects: HR, skin colour, mental status
resus for B
high flow oxygen via non rebreather mask with reservior bag
assessment for circulation
cardiovascular function, tissue perfusion, intravascular volume and control bleeding
- HR
- pulse volume
- cap refill time
- blood pressure
signs of poor circulation
- effortless tachypnoea
- poor peripheral perfusion
- agitation or drowsiness
signs of cardiac disease/heart failure
gallop rhythm
cyanosis
absent femoral pulses
hepatomegaly
resus for C
high flow oxygen
IV or IO access with blood collection for Ix
bolus of sodium chloride and repeat as necessary
early consideration of ionotropes
control external bleedings
treat arythmias
assessment for D
conscious level AVPU
pupil size, symmetry and rectivity
posturing (decorticate, ddeceberate) and tone
seizure activity
fontanelle
BGL
resus for D
consider intubation if response to pain or unresponsive
treate raised intracranial pressure (mannitol or sodium chloride 3%)
correct BGL
treat status elipticus
assessment for exposure
fully expose the child
- temperature
- abdominal examination
- non-blanching rashes or purpura
- urticaria or angiodoema
resus for exposure
aim for normothermia
antibiotics for suspected sepsis
treat anaphylaxis
analgesia
secondary assessment
obtain history
detailed physical examination
perform laboritory investigations and imaging
establish working diagnosis andd administer antibiotics for suspected sepsis
ongoing assessment
ECG monitoring
pulse rate
respiratory rate
oxygen saturations
blood pressure
temperature
monitoring devices that may be indicated
endotracheal tube carbon dioxide
venous blood gas
urine output
invasive blood pressure monitoring
central venous pressure monitoring
intracranial pressure monitoring
