FABULO (not right order)
1) high flow Oxygen
2) IV Fluid resuscitation
3) take Blood cultures
4) IV antibiotics
5) measure Lactate (and FBC) - <1 normal, 1-2 concerning, >2 bad, >4 very bad
6) monitor hourly Urine output
what are common causes of community acquired sepsis
E. Coli
S. Pneumonia
S. Aureus
what are common causes of hospital acquired sepsis
E. Coli S. Aureus Coagulase -ve staphylococci Enterococci Klebsiella spp. Pseudomonas spp
how do you treat a large abscess
drainage
how do you treat a small abscess
antibiotics
what is sepsis
systemic inflammatory response to infection
SIRS + signs of atlas 1 acute organ dysfunction
decrease in circulatory blood volume due to endotoxins causing vasodilation and intravascular fluid leaking into adjacent tissues
what is severe sepsis
acute organ dysfunction secondary to sepsis
what is septic shock
severe sepsis + hypotension refractory to adequate volume resuscitation or requiring vasopressors or inotropes
what are some local signs of sepsis
pain
tenderness
guarding
blood PR
what are some systemic symptoms of sepsis
fever rigors N and V constipation/diarrhoea malaise anorexia lethargy
what is the treatment of severe sepsis + enteric fever
ceftriaxone
what is SIRS
2+ of temp > 38 < 36 HR > 90 RR > 20 WBC > 12000 or < 4000
what are some s/s septic shock (meningitis)
non-blanching rash
headache
neck stiffness
strong BOUNDING pulse of 100bpm
