Sepsis Flashcards
Cardiovascular symptoms of shock
decreased cardiac output
increased HR
decreased BP
flat neck veins
Respiratory symptoms of shock
increased RR
shallow breathing
Kidney symptoms of shock
decreased urine output
Sepsis vs Septic Shock
sepsis is life threatening organ dysfunction brought on by infection
septic shock is the last most severe stage of sepsis itself
Stages of shock
Initial
compensatory
progressive
refractory
Initial stage of shock
decrease in MAP 5-10
mild vasoconstriction
increased HR
Compensatory stage of shock
Decreased in MAP 10-15
moderate vasoconstriction
increased HR
decreased pulse pressure
chemical compensation
decreased urine output
Progressive stage of shock
decrease in MAP >20
anoxia
hypoxia
tissue ischemia
Refractory stage of shock
severe tissue hypoxia with necrosis
multiple organ dysfunction syndrome
Risk factors to sepsis
malnutrition
80 years old or older
open wounds
CKD
HIV
s/s of shock
tachycardia
hypotension
increased cardiac output
decreased urine output
high lactic acid
Hour 1 bundle for sepsis management
1.) measure lactate
2.) obtain blood cultures before giving antibiotics
3.) give broad spectrum antibiotics
4.) begin rapid administration of 30 ml/kg of crystalloid
5.) vasopressors if fluid resuscitation did not work
