Diagnosis
Scurvy
Precipitating factors.
Pregnancy, lactation, and thyrotoxicosis; most common in alcoholism.
after how many months does scurvy occur after cessation of Vit C intake?
1-3 months after
Skin lesionsy
Petechiae, follicular hyperkeratosis with perifollicular hemorrhage, especially on the lower legs. Hair becomes fragmented and buried in these perifolliculár hyperkeratotic papules (corkscrew hairs,) also, extensive ecchymoses which can be generalized.
Nails: splinter hemorrhages
Sternum sinks inward: Scorbutic rosary
Gingiva lesions
Swollen, purple, spongy, and bleeds easily. Loosening and loss of teeth
Labs.
Normocytic, normochromic anemia. Folate deficiency, resulting in macrocytic anemia
Positive in what type of test.
Capillary fragility test.
Serum ascorbic acid level zero.
Treatment
Ascorbicacid 100mg three to five times daily until 4g is given;then 100mg/day is
curative in days to weeks
