who gets screening?
all women age >25 q3y who are or have ever been sexually active
what age has highest recommendation?
age 30-69
pap smears for age >70?
if 3 normal tests in row and no abnormal tests in last 10 years, can discontinue screening
if you get normal pap?
repeat in 3 years
if inadequate sample?
repeat cytology in 3/12
if adequate sample but no transitional zone?
repeat in 3 years
ASCUS: abnormal squamous cells of unknown significance in women <30 or no HPV testing?
repeat cytology in 6/12 twice
if positive at either then colposcopy
ASCUS: abnormal squamous cells of unknown significance in women >30?
HPV-DNA testing
if neg: repeat cytology in 12/12
if pos: colposcopy
screening detects ASC-H? abnormal squamous cells cannot rule out HSIL
colposcopy
screening detects AGUS - atypical glandular cells of unknown significance/atypical endocercial cells or endometrial cells?
colposcopy +/- endometrial sampling
screening detects High grade squamous intraepithelial lesion HSIL?
colposcopy
screening detects squamous carcinoma/other malignant changes?
colposcopy
screening detects Low grade squamous intraepithelial lesion LSIL - 2 options
- colpscopy immediately
- repeat cytology in 6/12 x 2 times: if neg: again
- if positive at any of the two then colposcopy
prostate cancer screening?
PSA not recommended for any age group
