SCLC, thymic, mesothelioma Flashcards
Limited stage SCLC treatment
Surgery followed by adjuvant cis/etoposide for T1-2 N0
Definitive chemoRT with cisplatin + etoposide for all others
*Don’t give G-CSF with chemo
1st line extensive stage SCLC treatment
Cis/carbo + etoposide + atezo/durva
Followed by maintenance immunotherapy
Consider prophylactic cranial irradiation and/or consolidative thoracic radiation
2nd line extensive stage SCLC treatment
Consider re-treatment with platinum + etoposide if relapse > 6 months after finishing 1st line
Topotecan
Lurbinectedin
Other single agent chemo
Thymoma/thymic carcinoma management
Surgical resection if able
Cisplatin-based chemo if not resectable or if recurrent
Mesothelioma chemotherapy
Cisplatin + pemetrexed +/- bev
Ipi/nivo
Management of localized bronchopulmonary typical/atypical carcinoid
Resection + mediastinal node sampling
No adjuvant therapy for low grade (typical carcinoid)
Consider adjuvant cis/carbo + etoposide or temodar for stage III intermediate grade (atypical carcinoid)
Management of unresectable bronchopulmonary typical carcinoid (low grade)
Observation if asymptomatic Octreotide/lanreotide if SSR+ Everolimus Temodar +/- capecitabine RT
Management of unresectable bronchopulmonary atypical carcinoid (intermediate grade)
Observation if asymptomatic RT +/- concurrent cis/carbo + etop Cis/carbo + etop Temodar +/- capecitabine Octreotide/lanreotide if SSR+ Everolimus
