Bosutinib dose and AEs
400mg daily
Diarrhea, pleural effusion
Dasatinib dose and AEs
100mg daily
Pleural effusions, pulmonary HTN, bleeding
Nilotinib dose and AEs
300mg BID
Prolonged QT, sudden death, pancreatitis, increased glucose, peripheral arterial occlusive disease
Imatinib dose and AEs
400mg day, can increase to 600-800
Fatigue, MSK pain, diarrhea, edema, cytopenias
3 month TKI failure criteria
Non-complete hematologic response and/or Ph+ >95%
If BCR-ABL1 >10%, switch TKI or increase imatinib dose
6 month TKI failure criteria
BCR-ABL1 > 10% and/or Ph+ >35%
12 month TKI failure criteria
BCR-ABL1 > 1% and/or Ph+ > 0
Accelerated phase criteria
10-19% blasts in blood or marrow; >= 20% blood basophils
Accelerated phase treatment
Start with higher dose of 2nd gen TKI
Dasatinib 140mg, nilotinib 400mg BID, or bosutinib 500mg
Ponatinib dose and AEs
45mg daily
