What is the difference between an SAI event and an LAI event?

A

Generally an SAI event has Brigade-wide implications while an LAI event has local implications.

“The main difference between LAI and SAI investigation is the development of an action plan at corporate level in the organisation without the need for it to go through the station/borough planning process.” (SAI manual)

What is an SAI?

A

A process which promotes a logical, consistent and professional approach to the investigation of safety events following systems failures that have resulted in our had the potential to cause death, serious injury or major financial loss to the authority, while providing an impartial and independent evaluation of events.

What events will involve the SAI process?

A

Events that place, or have the potential to place, the authority at significant risk. These involve failures that impact across the authority with the potential to place employees ar risk.
Eg. A failure in an appliance braking system
BA set malfunction resulting in a failure to supply air to the wearer at an incident
Failure of a 135 ladder

What are the three levels of accident investigation within the authority?

A
  • Senior Accident Investigation
  • Managed local accident investigation carried out by either an SAI or an LAI, with an SAI overseeing the investigation and reporting process
  • Local accident investigation where the investigation is dealt with by a qualified LAI (uniformed of FRS) who has undergone the authority LAI training

What should you do if a safety event appears to result from a willful disregard of procedures or might otherwise result in disciplinary procedures?

A

Contact the Head of Service/duty AC to seek immediate guidance.

Should you ‘reconstruct’ an event at the scene to identify what happened?

A

Reconstructions can jog people’s memories and provide further evidence. However, evidence has been lost due to well-intentioned Reconstructions. Dtw is a good example of this. Care should be taken to avoid any loss of evidence or any further injury or damage.
