Rosh Renew Flashcards
Dengue Fever
recent travel to tropical regions
high fever with a biphasic pattern, myalgias, and backache
thrombocytopenia, elevated LFTs and leukopenia
Most commonly caused by Aedes aegypti mosquito
Treatment is supportive
Malaria
- fever > 38.5°C (101.4°F) in a traveler
- classic triad fever, thrombocytopenia, and splenomegaly. -Fever is periodic.
- confirmed with blood smear or rapid detection tests.
- Treatment is supportive and targeted towards eradication of the protozoa.
Bacterial Meningitis Adult Rx
18 - 50 years: ceftriaxone + vancomycin
> 50 years: ceftriaxone + vancomycin + ampicillin (to cover Listeria)
Alcoholic Ketoacidosis
- In alcoholics
- decreased food and liquid ingestion leading to dehydration –> anion gap metabolic acidosis
- Patients require a nutritional source and need glucose or dextrose administration
The murmur of hypertrophic cardiomyopathy is accentuated by actions that decrease blood volume in the left ventricle and increase left ventricular outflow obstruction. These actions include:
the Valsalva maneuver or standing.
What effect does the Valsalva maneuver have on the murmur associated with aortic stenosis?
It decreases the murmur.
What are three organisms responsible for “atypical” pneumonia?
Mycoplasma, Chlamydia, and Legionella.
In patients with aortic dissection, what is the recommended blood pressure goal?
systolic blood pressure of 110 mm Hg.
What is digoxin used for?
Its positive-inotropic and antidysrhythmic effects, at therapeutic doses, are used in treating atrial fibrillation and congestive heart failure.
