features of gout?
swollen hot rapid onset erythema gout tophi in skin
common sites for gout?
first MTP
ankle
knee
what crystals are deposited in gout?
urate crystals
Ix for gout and results?
FBC (inflam markers raised)
serum uric acid (repeat once resolved)
aspirate and microscopy
what is found on gout microscopy?
needle shape with negative birefringence
Tx for gout?
NSAIDs/colchicine
analgesia
allopurinol (2-3wks post)
who gets allopurinol?
first attack >2 in a year renal disease diuretic Hx of renal calculi tophy
what causes pseudogout?
calcium pyrophosphate
where can pseudogout occur? how can it present?
knee
wrist
ankle
present as worsening OA
tender
Ix for pseudogout?
aspiration and microscopy
XRAY
Ca, PTH, Fe, Mg, Alk Phos
TFTs
what is found on microscopy for pseudogout?
rhomboid crystals with weakly positive birefringence
Tx for pseudogout?
NSAIDs/colchicine
rehydrate
