they may be hard copes in full (a ‘pack of’ hand written notes with paper copies of medical history, previous trt plans, consent forms). or may be fully computerised + only accessible on screen or a combination of both.

How are they collected?

A

by the dental nurse + are presented in assessment area so operator can check + access.

In a large workplace

A

nurse can check correct pt by identify by name, dob + address confirmation + if they know the reason for their attendance. The final decision to conform pt identity is the duty of the operator

A

may be recognisable by face but identity + procedure checks should still be carried out to avoid mishaps

Once identified, the nurse can prepare:

A

notes written at previous appts, medical history check, dental + periodontal, ortho charting may be separate paper inserts (hard copy) or drop down files (computer) to be accessed with ease, study models may be separate box + should be retrieved + set out for operation.

Prepare radiographs

A

relevant radiographs - hard copies correctly mounted on viewing screen:
oral views with pimple facing out indicate Left, right, upper or lower,
Bitewings pimple facing out + molar teeth on outer edge to indicate quadrants
Extra oral views with L marker in the LR corner of radiograph to indicate pt left side
