Smoking history
relationship to the incidence of oral cancer
Alcohol consumption
relationship to oral cancer
History of herpes infection
e.g primary herpes infection or cold sore as they are infectious to dental team
History of HIV
infectious + hazardous to dental team
History of Hepatitis
although vaccinated against hep B, other variations are infectious + hazardous to dental team
Diabetes type 1 or type 2
will be prone to xerostomia, oral infection + poor wound healing
Epilepsy
side effect of some medications used for this condition allows gingival hyperplasia to develop which affect their ability to clean their teeth effectively
Hypertension + heart disease

History of stroke
taking an anticoagulant e.g aspirin or warfarin - will need great care for extractions or minor oral surgery
Eating disorders
self induced vomiting result in enamel erosion + soft tissue burns
Digestive disorders
e.g croons disease + ulcerative colitis often manifests as oral ulceration in pts
Responsibility
of the operator to check medical history form + question pt further to clarify any medical issues that have been highlighted in its initial completion or update
