This should include:
- 6 point pocket depth measurements (ideally this sites >5mm highlighted)
- bleeding on probing (red dot above pocket measure)
- Recession
- Furcation involvement (I,II,III,IV)
- Mobility score (I,II,III)
How?
By dividing mouth into sextants + recording the presence + depth of any unnatural spaces down the side of teeth (periodontal pocket) - BPE assessment
Healthy periodontal tissue
appear pink, firmly attached to necks of teeth with gingival crevice no deeper than 3mm + don’t bleed when touched. Teeth firmly held in their sockets by periodontal supporting tissues, + no plaque present
Basic Periodontal Exam (BPE)
This index formerly known as CPITN is measured by WHO probe. is introduced into gingival sulcus + light pressure used around buccal, lingual + palatal. The mouth is divided into sextant represented by box charts and only the highest score is recorded

healthy gingival tissues with no bop
Code 1
BOP, black band remains visible as pocket depth no more than 3.5mm. no calculus or other plaque retention factor present.
Code 2
black band visible as pocket no more than 3.5mm but plaque retention factor detected
Code 3
black band partially visible (shallow pocket 5mm) in deepest pocket as up to 5.5mm may not be visible
Code 4
pocket deeper than 5.5mm, furcation involvement and gingival recession added to probing gives depth of 7mm or more

Higher codes
indicate more serious periodontal problem, codes greater than 3 are recorded, full periodontal assessment, presence of bleeding, plaque found + tooth mobility
Tooth mobility grades
grade I - side to side tooth movement less than 2mm
grade II- side to side tooth movement more than 2mm
grade III - vertical movement present
