What is it?
a diagrammatic representation of teeth and their surfaces providing a visual note of the patient’s teeth and any trt they have had.
Why is it important?
shows the dentist an immediate representation of the pts mouth, diagnostic for treatment and for forensic identification of bodies.
What do they show?
Teeth present, teeth missing, work to be carried out, work completed, surfaces with cavities restorations etc
Charting
the mouth is looked on as being a flat line, at pts first visit they will have a full base chart recorded and should be checked every time they attend
Palmer tooth notation
quadrants are referred as UR,UL,LL,LR as well as the individual teeth are numbered 1-8
The international dental federation two digit system
is globally recognised form of tooth notation where quadrants are recognised as numbers 1-UR, 2-UL, 3-LL, 4-LR and for deciduous teeth: 5-UR, 6-UL 7-LL, 8-LR
FDI
The quadrant symbol is replaced by a number, the quadrant number is the first digit and the second number identifies the individual tooth.
On a dental chart
separates existing restorations from those that are required. The inner grid is present dental status and work already present. The outer grid is for work to be carried out
Incisal
the biting edge of the incisor and canines
Occlusal
the biting surface of premolar and molar
Mesial
the surface nearest to the midline of the arch
Distal
the surface furthest from the midline of the arch
Buccal
surface facing the cheeks on molars and premolars
Labial
surface facing the lips on incisors and canines
Palatal
surface facing the palate of all upper teeth
