where to do pleural tap?
9th intercostal space in MAL during expiration
When there is a pleural effusion, the lung will be collapsed.
Hence, these locations would also be acceptable:
- 5th intercostal space at the MAL-pleural effusion/tension pneumo
- 2nd intercostal space at the MCL – emergency Tension pneumothorax
where does foreign stuff fall into ?
goes down right main bronchus into lower lobe
supine pt: apical lower lobe (aka Superior segment of the inferior lobe)
standing pt: posterior basal segment of the lower lobe (aka Inferior segment of the inferior lobe of the right lung)

thoracentesis/pleural tap
Insert the needle into the 9th intercostal space in the MAL during EXIRATION to avoid the inferior border of the lung.
needle is inserted superior to the rib
needle should be angled upward to avoid penetrating the deep side of the recess (such as the diaphragm)
