Gold standard investigation of choice for sarcoidosis along with other investigations necessary

A

Histology via bronchoscopy with ultrasound guided biopsy of mediastinal lymph nodes

Bloods: raised ace, hypercalcemia, raised interleukin 2receptor, raised crp,

Chest X-ray showing hilar lymphadenopathy

Check for organ involvement with Lfts,u and e, ophthal review etc

Tx of choice

A

First line: no treatment

Oral steroids: dexa and orednisolone given for between 6&24 months with bisphosphonates to protect against osteoporosis with long term steroids

Second line: immunosuppressive such as azathioprine and methotrexate

Lung transplant
