what is diabetic nephropathy
A
presence of microalbuminuria or overt nephropathy(macroalbuminuria) in a patient with dm and has no other cause of their renal disease

what is the screening recommendation for diabetic nephropathy
A
serum creatinine
assess albumin to creatinine ratio
ACR> 2.0 in men/>2.8 in women the microalbuminuria
ACR> 20 in men/>28 in women then macroalbuminuria

what is the approach to treatment in a patient with diabetic nephropathy
A
glycemic control
bp control with acei or arb 2nd line ca channel blocker eg diltiazem or verapamil if unable to tolerate acei/arb
limit nephrotoxic drugs
possible renal transplant and hemosyalisis if renal failure
