Pulmonary 4 Flashcards
Target of AC/VC
Flow
Target of AC/PC
Inspiratory pressure
What causes a PC breath to cycle off?
Time
Types of breaths in volume SIMV
VC, VA, PS, spontaneous
Types of breath in pressure SIMV
PC, PA, PS, and unassisted
Why is there a pressure plateau at the end of each AC/VC breath?
End-inspiratory pause (because it is time-cycled)
ATS/IDSA criteria for severe CAP (major criteria)
Needs pressors or vent
2 scenarios is which empiric MRSA coverage is recommended for Severe CAP
- Prior respiratory MRSA
2. Parenteral Abx in last 90 days + MRSA risk factors
How many minor criteria are needed to Dx severe CAP?
3
Minor criteria for severe CAP (9)
RR ≥30/min, P/F ≤250, multilobar infiltrates, confusion, uremia, leukopenia, thrombocytopenia, hypothermia, or hypotension requiring aggressive fluid resuscitation
Usual empiric Tx for severe CAP
β-lactam plus a macrolide or a β-lactam plus a fluoroquinolone
Which are the atypical organisms for CAP?
Legionella, Mycoplasma, and Chlamydia
Does Zosyn cover atypicals?
No!
