2 main indications for exchange transfusion in acute chest syndrome
- severe hypoxemia (SpO2 ≤85% or PaO2 ≤60); 2. multilobar involvement
What is reverse triggering?
mandatory breath causes pt to reflexively inhale (initiating a second breath)
What is delayed cycling?
inspiratory time is too long for patient, so they start to exhale while positive pressure breath is still being delivered
What does delayed cycling look like on vent pressure waveform?
spike in pressure at the end of breath delivery
What does delayed cycling look like on vent flow waveform?
reversal of flow toward the end of delivered breath
Proper ETT position relative to the carina
3-6 cm above the carina
