Time limit for TPA for stroke in pts under 80
4.5 hours
Time limit for TPA for stroke in pts over 80
3 hours
Time limit for mechanical thrombectomy for stroke (per 2017 guidelines)
6 hours
Time limit for mechanical thrombectomy if large vessel stroke with significant reperfusible brain on angiogram by RAPID software estimate
16-24 hours
Only Tx for both cerebral salt wasting and SIADH
Hypertonic saline
2 meds for hypotension in Guillain-Barre
Fludrocortisone and midodrine
_____ is mandatory if SAH is suspected, but CT head is negative.
Lumbar puncture
Next step once SAH is diagnosed (and pt is stable)
Identify the source of bleeding
What are 2 non-traumatic sources of bleeding in SAH
Aneurysm and AVM
3 tests to identify source of bleeding in SAH
Digital subtraction angiography (DSA), CTA, and MRA
If SAH confirmed but no evidence of cerebral aneurysm or other source of bleeding in the brain, do this.
Look for spinal AVM (e.g. with spine MRI)
Location of stroke required for mechanical thrombectomy at <6 hrs
internal carotid or MCA
