What aspects of neurobiology are associated with OCD?
Frontal subcortical activity (motivations), abnormal basal ganglia connections (routine behaviors) and dysregulation of 5HT, DA and Glu.
A 10 year old girl comes to the clinic with rapid onset OCD in the past ten days. What is a possible etiology of her condition?
She has PANDAS. This is rapid onset of OCD in kids with a recent group A strep infection and presence of anti-basal ganglia antibodies.
How do you treat patients with OCD?
SSRIs, exposure response prevention therapy, surgical capsulotomy.
A woman comes to see you complaining of a messed up nose when her nose looks fine. She constantly obsesses over it and covers her nose whenever someone looks at her. What is your diagnosis?
Body dysmorphic disorder. It is a repetitive behavior in response to an obsession with physical appearance.
When do you usually see people present with this?
Trichotillomania presents around puberty when hair begins to develop in other parts of the body, it tends to regress later in life.
How will this patients disease progress and he get older?
Hoarding disorder tends to get worse as people get older.
What is your diagnosis?
Excoriation disorder. This often begins when people get acne and pick at zits.
What conditions are common comorbidities with OCD?
MDD, anxiety
