the present condition of teeth is recorded on dental chart + any other conditions such as state of existing restorations + dentures, level of OH, periodontal disease, malocclusion + tooth discolouration, that may affect trt are also noted

A
pt’s general attitude towards their OH + accordingly advise the most appropriate trt. Must of this info is conveniently set out in Oral Health Assessment documents that are currently being piloted before NHS contracts change again in near future + can be used as prompts to gather required info

A
are asked + responses recorded about all issues that may affect pt’s OH, especially diet, alcohol consumption + tobacco usage
