Listening to the heart
- Aortic (right 2nd intercostal space)
- Physicians (left 2nd intercostal space)
- Tricuspid (left 4th intercostal sternal border)
- Mitral (left 5th intercostal, midclavicular line)
Accentuation manoeuvres: auscultate over the carotid arteries with the diaphragm of the stethoscope whilst the patient holds their breath to listen for
radiation of an ejection systolic murmur
→ aortic stenosis
Accentuation manoeuvres: auscultate over the aortic area (right sternal border second intercostal) with the diaphragm of the stethoscope during expiration to listen for an
early diastolic murmur
aortic regurgitation
Accentuation manoeuvres: roll patient on their left side and listen over the mitral area with the diaphragm of the stethoscope during expiration to listen for a
pansystolic murmur
mitral regurgitation.
Accentuation manoeuvres: roll patient on their left side and listen over the mitral area with the bell of the stethoscope during inspiration to listen for a
mid-diastolic murmur caused by mitral stenosis.
