Begins within 48 hours delivery. Peaks at 3 to 5 days. Last around 2 weeks.
Postpartum Blues
Sx include: mood lability, tearfulness, sadness, anxiety, irritability, insomnia, decreased concentration
postpartum blues
Treatment of postpartum blues
support, rest and monitoring for development of postpartum depression
onset within 2 weeks of delivery. More likely to committ suicide or infanticide. Thought of as a first manifestation of bipolar disorder.
postpartum psychosis
Sx include: delusion, hallucinations, thought disorganization, severe insomnia, aggression, agitation, impulsivity
postpartum psychosis
Treatment for postpartum psychosis
hospitalization, antipsychotics, therapy
Can present up to 4 months postpartum and there’s an increased risk for up to one year postpartum
postpartum depression
Sx include: feelings of inadequacy, guilt, hopelessness, being overwhelmed, not bonding with baby
postpartum depression
Positive score on Edinburgh Postnatal Depression Scale
12 or more is positive. 5-9 re-evaluate in 1 month
Preferred medications for post partum depression
SSRIs. Preferrably sertraline (Zoloft) and Paroxetine (Paxil)
SNRI option for postpartum depression
Duloxetine (Cymbalta)
Are benzos and sedative hypnotics (ambien/lunesta/sonata) safe for breastfeeding?
No. If going to be on long-term woman should stop breastfeeding
