What is PPH?
> 500ml blood loss per vagina after delivery of baby and placenta
What is a primary PPH?
Within 24 hours of birth
What is a secondary PPH?
24hrs - 2 weeks post delivery
What can cause secondary PPH?
Endometritis
Retained placental tissue
How is secondary PPH investigated?
USS
Swabs
How is secondary PPH managed?
Abx
+/- surgery
What is a major PPH?
> 1000ml blood loss
What is a minor PPH?
500-1000ml blood loss
What can cause PPH?
4 Ts
Tone = uterine atony Tissue = retained placenta tissue Trauma = perineal tear Thrombin = coagulopathy
What are risk factors for PPH
Previous PPH Multiple pregnancy Obesity Large baby Pre-eclampsa Instrumental delivery
How can PPH be prevented?
Treat antenatal anaemia
Give birth with an empty bladder
Active management of the third stage
How is PPH managed generally?
A to E
How is PPH due to tone managed?
Bimanual compression
Oxytocin - increase contractions
Surgical methods
- intrauterine balloon
- B-lynch suture around uterus
How is PPH due to trauma managed?
Repair tear
How is PPH due to tissue managed?
IV oxytocin + manual removal
