What is placental abruption?
When all or part of the placenta separates from the wall of the uterus prematurely
What is the pathophysiology of placental abruption?
Rupture of maternal vessels in basal layer of endometrium
Blood accumulates + splits basal layer and placenta
What is a revealed placental abruption?
Blood tracks down -> vaginal bleeding
What is a concealed placental abruption?
Bleeding remains in uterus
Often present with signs of hypovolaemic shock
What are risk factors for placental abruption?
Previous placental abruption
Pre-eclampsia
Abnormal lie
Abdominal trauma
What are features of placental abruption?
Painful vaginal bleeding
Tense woody uterus
How is placental abruption managed?
Maternal/fetal compromise

- Emergency delivery
Induction of labour
Small bleed = conservative
