What is population health ?
the health outcomes of a group of individuals, including the distribution of such outcomes within the groups
What is the goal of the Healthy People Framework ?
gives a 10-year framework that is science-based to improve the health of all Americans
What are the 5 goals in the Healthy People Framework ?
- attain healthy, thriving lives and well-being free of preventable disease, injury, or death
- eliminate health disparities, achieve health equity, and attain health literacy to improve the health and well-being of all
- create social, physical, and economic environments that promote attaining the full potential for health and well-being for all

What are Health Disparities ?
systematic, potentially avoidable health differences that happen based on social disadvantages or social advantages
- poverty, racial/ethnic disparities, children and elderly are examples of vulnerable populations
What is the leading health indicator ?
infant mortality
- can be linked to factors like maternal health and access to healthcare
What are the components of the social determinants of health ?
- neighborhood and built environment
- are there sidewalks where you can walk ?
- do you feel safe to send your kid outside to play ? - health and health care
- economic stability

What does the social determinants of health mean ?
the circumstances in which people are born into, live, work, and age as well as the systems put in place that affect health
What is health equity ?
where everyone has a fair and just opportunity to be as healthy as possible
- in a “perfect world”
- where there is an absence of systematic disparities in health between social hierarches
What are ways to resolve health disparities ?
- federal policy
- ACA, medicare & medicaid, Healthy People - state policy
- Hoosier Healthwise - community programs
