placenta is attached in the lower portion of the uterus, lower than the presenting part of the fetus
associated factor with placenta praevia
multiparity
multiple pregnancy
previous caesarean section
clinical features of placenta praevia
shock in proportion to visible loss
no pain
uterus not tender
lie and presentation may be abnormal
fetal heart normal
small bleeds
diagnosis of placenta praevia
digital vaginal exam NOT PERFORMED before USS
is often picked up on routine 20 weeks abdo USS
-transvaginal ultrasound
management of placenta praevia
rescan at 32 weeks
- again at 36 weeks if still present
give corticosteroids
planned delivery between 36 and 37 weeks
emergency caesarean for premature labour or antenatal bleeding
