  • PFA Failure notification (written):
    o Enlisted Sailor must be provided a written PFA notification within 30 days of the command’s completed PFA cycle
    o Officers must receive LON within the same amount of time
  • Enroll member into FEP until next PFA is completed
  • Member (officer or enlisted) is not permitted to be frocked or advanced until mock or official PFA is completed SAT
  • If enlisted Sailor’s PFA is failed for the 2nd time in a row:
    o Continue service until re-enlistment
    o Ineligible for advancement
    o Ineligible to re-enlist or extend service
    o Will receive an ADVERSE Eval with SP
  • If enlisted Sailor’s PFA failed is the 2nd within 3 years:
    o EVAL “Military Bearing” or “Professionalism” -> 1.0
    o Mark SP and RETENTION NOT RECOMMENDED
    o Include a specific statement explaining the grade in the Block 41/43 writeup
  • If officer’s PFA failed is the 2nd (consecutive) failed PFA:
    o Submit the officer for ADSEP processing
    o If SECNAV determines the officer is to be separated, this action will occur at the PRD of the officer
    o Receive an adverse FITREP documenting the failures
    o If an officer passes an official PFA prior to the decision of the SECNAV on retention or separation, processing will cease and the member will be retained upon notification to PERS-834. A special FITREP may be submitted.
  • If officer’s PFA failed is the 2nd PFA within 3 years:
    o FITREP “Military Bearing” -> 1.0
    o Mark for promotability is SP
    o Include a specific statement explaining the grade in the Block 41/43 writeup
