What is the definition of a Class A Mishap?
Total cost of reportable material or property damage is $2 million or more, or injury or occupational illness that results in fatality or permanent total disability
What is the definition of a Class B Mishap?
Total cost of reportable material or property damage is $500,000 or more, but less than $2 million, or injury or occupational illness that results in permanent partial disability or three or more personnel are inpatient hospitalized
What is the definition of a Class B Mishap?
Total cost of reportable material or property damage is $50,000 or more, but less than $500,000, non-fatal injury, causes any loss of time beyond the day/shift on which it occurred, non-fatal illness or disease that cause loss of time from work/disability at any time.
Reporting purpose: mishaps are those which result in five or more lost work days beyond the date of injury or illness
