What is Stage A heart failure?
High risk patients with: Hypertension CAD Diabetes mellitus Family history of cardiomyopathy
What is Stage B heart failure?
Asymptomatic HF
PRevious MI
LV systolic dysfunction
Asymptomatic valvular disease
What is Stage C heart failure?
Symptomatic HF
Known structural heart disease
Shortness of breath with fatigue
Reduced Exercise tolerance
What is Stage D heart failure?
This is refractory/end stage HF
with marked symptoms at rest!
What drugs are indicated for the use in Stage A-D heart failure?
ACEi and ARBS
What drugs would be indicated in recent or remot MI with asymptomatic Left ventricular disease?
ACEi or ARB with a Beta blocker.
This is seen in Grade B heart failure.
What medication combinations are absolutely indicated for Grade C heart failure?
Acei or ARB
with a Beta Blocker.
What medication should be used in a symptomatic LVD patient thats in stage C heart failure?
Aldosterone antagonists.
Dont forgit though that all Stage C patients should be on an ARB or ACEi with Beta blockers.
What drug concoction is an excellent choice for African Americans experiencing heart failure?
Hydralazine and nitrates.
What should be given in a patient experiencing symptomatic HF with atrial fibrillation?
Digoxin
