How do the drugs Acetazolamide and Dorzolamide work?
These are carbonic anhydrase inhibitors that prevent the absorption of bicarbonate and subsequently sodium.
they do this by inhibiting the intracelluar carbonic anhydrase decreasing the generation of H+ needed for the Na/H+ channel.
Which part of the kidney do carbonic anhydrase inhibitors work in the kidney?
Work at the proximal convuluted tubule.
What are the most common uses for the carbonic anhydrase inhibitors?
Glaucoma
Acute mountain sickness
Urinary alkinzation
Edema treatment when combined with NKCC or NCC inhibitors.
How does the drug Mannitol work
This is a osmotic diuretic that is freely filtered but poorly absorbed.
This leads to increased tubular fluid osmotic pressure and a decreased tubular fluid reabsorption.
Where does the diuretic mannitol work best?
Works best at the thin limbs of the loop of henle.
What are the best uses for mannitol?
Decrease intracerebral pressure in cerebral edema.
Also serves as prophylaxis for acute renal failure!!!
Can be used in glaucoma as well.
What are the side effects of the osmotic diuretics such as mannitol?
Mannitol can cause pulmonary edema in patients with heart failure.
Can also cause acute hypobolemia.
When should you not use mannitol?
Contraindicated in active cranial bleeding (mannitol and urea)
