What are indications for anti-convulsant therapy?
If the pt doesnt respond or cant tolerate lithium
What is Carbamazepine (Tegretol) used for?
Bipolar and major depression
How long does it take for tegretol to start working?
1 month for full effect
What are side effects for Tegretol?
Sedation, lethargy, blurred vision- diminish over time.
What is monitored for Tegretol?
Liver enzyme checks, WBCs, and Ataxia is a side effect of toxicity.
What is Valproate (Depakote) used for?
Acute mania
What is Depakote contraindicated in?
Pregnancy- causes neural tube defects.
What are side effects of Depakote?
GI irritation, sleepy, tremor, weight gain, hair loss.
What should be frequently checked during Depakote use?
Liver enzymes.
What are some other drugs for Manic patients?
Gabapentin, Lamotrigene, Topiramate, Calcium channel blockers
What is some teaching on calcium channel blocker use?
Watch patient for signs of excessive vasodilation- dizziness and hypotension. Pts who are not depressed and are taking these meds can become depressed.
What should you watch when a pt is on Lamictal?
Skin rash= sign of Stevens-Johnson syndrome and flu like symptoms.
What is Stevens-Johnson Syndrome?
Topical dermal necrolysis- Severe, fatal allergic reaction that affects skin, mucus membranes, lungs, eyes, and kidneys.
How do you manage a patient with Stevens-Johnson Syndrome?
Burn center- hydrate, parenteral nutrition, steriod therapy
