Peptic Ulcer Disease Flashcards
What is peptic ulcer diease?
Focal destruction of gastric mucosa and small intstine, mainly the prox. duodenum caused by gastric secretions
Typical sites of PUD?
Distal stomach and proximal duodenum
What are the two major contributors to PUD?
HCl secretion and H. pylori infection
What does acidification of the duodenal bulb lead to?
Islands of metaplastic gastric mucosa in the duodenum in many patients with PUD
What is necessary for formation and persistence of peptic ulcer disease?
Hyperacidity
RFs for PUD?
- Diet
- Drugs (NSAIDs)
- Smoking
- Genetic factors
What blood group is associated with PUD?
Type O
What is Pepsinogen I’s role in PUD evolution?
High pepsinogen I levels has five times the normal risk of developing a duodenal ulcer
What determines gastric acid production?
Function of total parietal cell mass
What is the role of accelerataed gastric emptying in duodenal ulcers?
Lead to excessive acidification of the duodenum
What diseases are associated with Peptic Ulcers?
- Cirrhosis
- Chronic Renal Failure
- Hereditary Endocrine Syndromes
- MEN1
- Zollinger-Ellison Syndrome
- Alpha1 Antitrypsin
What does a chronic peptic ulcer resemble?
Gastric Carcinomas
Classic presentation of a duodenal ulcer?
Epigastric pain 1-3 hrs after a meal
What is a life-threatening complication of active PUD?
Massive hemorrhage
What is the condition that occurs following a perforation of the stomach leaking air into th abdominal cavity?
pneumoperitoneum
