there are 3 compartments - this is prolapse of all 3
What is a cystocele?
bladder prolapse - drops into vagina
anterior compartment
What are the symptoms of a cystocele and enterocele?
incomplete emptying, pressure, bulging, difficulty voiding
painful or difficult intercourse, tampon insertion
dragging sensation
What is an enterocoele?
vaginal vault
middle compartment
What is a rectocele?
prolapse of front wall of bowel into vagina
posterior compartment
What are the symptoms of a rectocele?
difficulty pooping incomplete pooping bulging pressure mass splinting vaginal wall
How are pelvic prolapses investigated?
examination
history - sexual dysfunction, urinary and bowel symptoms, pressure, dragging sensation
USS, MRI
What is the treatment of pelvic prolapses?
pessaries - for mild to moderate, families incomplete, frail patients
surgery
What are the complications of pessaries for pelvic prolapse?
fibrous band formation
ulcerations
How often should pessaries for pelvic prolapse be changed?
every 6 months
What is urinary incontinence?
any involuntary leakage of urine
can be stress, urge, mixed, overactive bladder
What is stress incontinence?
leakage on effort
e.g. sneezing, cough, laughing
What is urge incontinence?
leakage preceded by urgency
What is mixed incontinence?
urgency and on exertion/sneezing etc
